Registration Information

NEW STUDENTS

All new Sycamore students must complete a school registration form, show a record of immunizations, and a copy of your utility bill.

If your child has special services, 504 Plans and IEPs need to be included with your registration information.

If your child has special medical needs please contact the school as early as possible to discuss any care your child may need during the school day.

It is the parents' responsibility to see that all forms and special requests are submitted prior to your child being permitted to attend Sycamore.
